Understand the Perform Log in Ausmed Perform

This article explains what the Perform Log is, how to use Perform Notes to capture internal performance observations, and who can access them.

What is the Perform Log?

The Perform Log in Ausmed Perform is a secure space for documenting internal performance observations about staff. These notes—called Perform Notes—are not visible to the staff member and are intended for internal use only.

Use Perform Notes to:

  • Record performance issues or incidents

  • Document examples of behaviour, professionalism, or attendance

  • Track informal feedback or coaching moments

  • Build a consistent performance record to support future reviews or HR decisions


How to add a Perform Note

You can add a Perform Note in two main ways:

1. From the Perform Log

Steps:

  1. Go to Ausmed Perform and click on Perform Log.

  2. Select Add Perform Note.

  3. Choose the individual the note is about.

  4. Select the date the observation occurred.

  5. Write your note using the text editor.

  6. (Optional) Attach evidence (e.g., a link to a document).

  7. Select:

    • Type of issue: Attitude/Behaviour, Performance, Professionalism, Time and Attendance, or Other

    • Action requested: Investigate, Pass to HR, Monitor, Other

    • Link to standards/capability frameworks (if relevant)

    • Sentiment: Positive, Neutral, or Constructive

  8. Save the note.

2. From the My Tasks tab
  1. Go to My Tasks.

  2. Click Add Perform Note.

  3. Complete the form as above.


How to review Perform Notes

To review notes:

  1. Go to Perform Log.

  2. Select the staff member.

  3. You’ll see a list of notes including:

    • Date of observation

    • Type of issue

    • Action requested

    • Sentiment

    • Who added the note and when

    • Any attached evidence

  4. Click View to see the full content of a note.


Who can see Perform Notes?

  • Organisation Managers and users with the appropriate permissions can view all notes across the organisation.

  • Managers can only view notes about staff they directly manage (i.e. their direct reports).

This helps ensure privacy and role-appropriate visibility.


Tips

  • Use clear, specific language in your notes to ensure context is preserved.

  • Avoid duplication by checking existing notes for repeated observations.

  • Regularly update the log if issues are ongoing or progress is made.

Note: Perform Notes are internal only. Staff will not be notified about these entries and cannot access them.
