What the Action Log is for

The Action Log lets you record the follow-up action taken in response to a Not Yet Competent (NYC) outcome, directly against the outcome itself.

The biggest risk after an NYC isn't the result itself — it's an NYC that sits there with no plan and no follow-up. That's the hardest scenario to defend at audit, and the easiest to let slip through the cracks, potentially risking an incident or harm.

With the Action Log, every NYC outcome can carry a clear, structured record of what was done in response, so managers can be confident nothing falls through, and the organisation has real evidence that a gap was found and acted on.


Adding an action

Steps

  1. Locate the NYC outcome record at any of the following locations:

    1. The competency dashboard
    2. The staff member's profile page
    3. The Outcomes Report
  1. Select Document action, and enter the date the action took place, then add a note describing what was done.

  2. Click Add to finish.

Viewing and updating actions

Locate the NYC outcome record to edit or delete entries you've added yourself. You can't edit or delete entries added by someone else, even if you have access to the same outcome.


Locating all actions in one place

The Action Log list page shows every note added across your organisation, one row per note. If three managers each add a note to the same outcome, all three show up here.

If there are no actions to show — or you don't have access — you'll see an empty state instead of the list.

You can download notes as a CSV for reporting or audit purposes — useful if you need to share your follow-up records with someone outside your organisation, such as an Auditor.
