This article walks you through how to add a self-assessment activity for use in performance reviews in Ausmed Perform. You’ll learn when and how to use self-assessments, available question types, and what happens after publishing.

What is a self-assessment?
Self-assessments are structured activities where the staff member reflects on their own performance. After submission, the manager reviews the staff member’s responses and adds comments.
Self-assessments are typically used in:
- Probation reviews
- Annual or bi-annual performance check-ins
- Professional development planning
Steps to add a self-assessment
- Go to Reviews > Activities
- Click Add Activity
- Choose Self-Assessment
- Enter a title and description (optional)
- Check whether you require a Manager Review or Auto Complete on Staff Submission
Require Manager Review ensures that a manager must review an employee’s self-assessment before it can be completed.
How it works

Employees submit their self-assessment as usual

The assessment remains Open and pending manager review

Managers are required to review the assessment

Managers can add comments during the review process
This setting reflects how self-assessments traditionally functioned in Ausmed Perform and is ideal where formal sign-off or feedback is required.
Autocomplete on Submission
Autocomplete on Submission allows a self-assessment to be marked as Completed immediately after the employee submits it.
How it works

Employees submit their self-assessment

The assessment automatically moves to Completed

No manager review is required

Managers can still conduct a meeting or discussion immediately
This option is useful when the self-assessment itself is the primary requirement and a formal review step is not needed.
- Add questions using the available formats (see below)
- Mark questions as required or optional
- Choose to Save as Draft or Publish

Customisation options
- Order your questions to guide staff through a clear reflection
- Mark questions as required if the answer is essential
- Toggle whether a Manager Comment Box should be enabled for the question
When adding a question in the self-assessment creator, you’ll see a toggle labelled Show manager comment box.

A manager comment box appears for that question during manager review

No manager comment box appears for that question

This allows you to control exactly where manager input is required, rather than applying comments to every question by default.

Question 1: Manager comment box disabled → only the employee’s response is shown

Question 2: Manager comment box enabled → employee response and manager comment box are both shown during review
- Save your work as a draft to preview or collaborate
- Once staff submit their self assessment, managers are required to:
- Review the staff’s responses
- Add additional comments before the activity is marked as complete
- Review the staff’s responses
Save as Draft vs Publish
- Draft: Allows you to continue editing. Can be previewed but not assigned.
- Publish: Locked in. Once published, it cannot be editedOnly published activities can be used in a performance review program.
Tip: Save your self-assessment as a draft first to gather feedback or test it with a small group.
