This article walks you through how to add a self-assessment activity for use in performance reviews in Ausmed Perform. You’ll learn when and how to use self-assessments, available question types, and what happens after publishing.

What is a self-assessment?

Self-assessments are structured activities where the staff member reflects on their own performance. After submission, the manager reviews the staff member’s responses and adds comments.

Self-assessments are typically used in:

  • Probation reviews

  • Annual or bi-annual performance check-ins

  • Professional development planning


Steps to add a self-assessment

  1. Go to Reviews > Activities

  2. Click Add Activity

  3. Choose Self-Assessment

  4. Enter a title and description (optional)

  5. Add questions using the available formats (see below)

  6. Mark questions as required or optional

  7. Choose to Save as Draft or Publish


Available question types

You can mix and match any of the following formats:

Customisation options

  • Order your questions to guide staff through a clear reflection

  • Mark questions as required if the answer is essential

  • Save your work as a draft to preview or collaborate

  • Once staff submit their self assessment, managers are required to:

    • Review the staff’s responses

    • Add additional comments before the activity is marked as complete


Save as Draft vs Publish

  • Draft: Allows you to continue editing. Can be previewed but not assigned.

  • Publish: Locked in. Once published, it cannot be editedOnly published activities can be used in a performance review cycle.

Tip: Save your self-assessment as a draft first to gather feedback or test it with a small group.
